SUBJECT: Request for Massage Therapist License
INTRODUCTION
Ross Bergfalk has made application for a Massage Therapist License. Mr.
Bergfalk has met the City's requirements for a massage therapist. Mr.
Bergfalk will be working for The Waterfall Tanning and Day Spa beginning
January 1, 1996.
