Staff has invited Hanna Klu~runek from the Central MN Housing Partnership to discuss their
Continuum of Care program. This program is a network of organizations, businesses and residents
that work together with a primary goal of alleviating homelessness in Central Minnesota.
Ms. Klimmek will be providing an overview of the program as well as opportunities for our
community to participate.

Continuum of Care Overview
The Central MN Continuum of Care (COC) is a network of organizations, community residents and
businesses that plan programs with the primary goal of alleviating homelessness in 14 counties in Central
MN, as well as the City of St. Cloud. The counties include: Cass, Wadena, Crow Wing, Todd, Morrison, Mille
Lacs, Kanabec, Pine, Isanti, Chisago, Stearns, Benton, Sherburne and Wright. Hanna Klimmek serves as the
Central MN Continuum of Care Coordinator.
Who is Involved in the Continuum of Care
Government officials, local public housing authorities, county officials and service providers from 14 counties
and the St. Cloud Area
What is the purpose of the Continuum of Care
The Continuum of Care serves two main purposes:
1. It is a strategic plan to address the use of HUD resources and their interface with other funding
sources, developed through acommunity-based process to address homelessness based on: the
identified needs of homeless individuals and families; the availability and accessibility of existing
housing and services; and the opportunities for linkages with non-homeless mainstream housing and
services resources. Through the Continuum of Care planning process, a community can:
o Identify the size and scope of the homeless problem;
o Inventory the resources available in the community to address the problem of
homelessness, including both homeless resources and non-homeless targeted housing and
service resources (referred to as "mainstream" resources);
O Rank the community's needs in order of priority;
O Strategically plan the range of services and housing that should be implemented to address
homelessness; and
O Identify available leveraging resources that can be used to address homelessness.
2. It is an application to HUD for McKinney Homeless Assistance resources. These resources are
Current Active Grants invaluable in providing housing and supportive services for people who are homeless. These funds
are made available through a national competition announced each year in HUD's Notice of Funding
Availability. The Central MN COC has secured $10,668,789 in funding since 1997 through this
application.
