ACTION REQUESTED
Motion to approve the following:
1. Renewal of a massage establishment license for Avalon Salon located at 706 Main Street.
2. Massage therapist license for Tara LeFebvre to work at Avalon Salon.
BACKGROUND/DISCUSSION
The owner of Avalon Salon was not initially planning on renewing her massage establishment license;
however, she has since hired a new massage therapist and has requested renewal of Avalon's
establishment license.
Ms.Lefebvre has applied for a massage therapist license to work at Avalon Salon. Ms.Lefebvre has been
licensed as a massage therapist at a local chiropractic clinic for the past few years;however,Ms. Lefebvre
will now be working for Avalon Salon.
All necessary renewal application materials have been received.
