Also, it is a review of OSHA and DOT Safety compliance of your program. The SP-9 evaluation format that I am using is detailed in your AWAIR Program. I have added a Program evaluation to the process given in the month of January 2004. The specific comments are to be used as a tool and a guide for our approach to strengthen your Safety Program in the following year. First evaluation is the (SP-9) review of your Safety Program consisting of 9 elements. The following is an overview of the Safety categories and your Safety Program evaluation in each category by me. 1. Employer Commitment: • Good Pro-active. Safety is a concern. 2. Employee Involvement: • Good attitude. Good participation. • The City Safety Committee tries to be very Pm-active and the Utility representation is a great asset. 3. Hazard Control: • Documented walk through self-inspections are done on a regular basis. • Written SOP and guidelines are needed and in the process of being completed by Glenn and the Staff. 4. Facility Maintenance: • Concerns are addressed as needed. 5. Accident Analysis: • Accidents are being reviewed. • Near mis::es are identified and reviewed at Safety mee.ings. 6. Occupational Medicine: • Hearing Screening is needed and scheduled for 2004. 7. Contractor Safety: • We need to continue to inform contractors of their Safety responsibilities. 8. Employee Training: • Compliance Training is on pace and documented. 9. Emergency Response: • All Departments kept well informed. The second means of evaluation is the MMUA Safety Program Evaluation/Comment Questionnaire. The entire City&Utility Staff rated the following questions, and here are the averages for each topic. Due to the way the evaluation was distributed and collected, it is an evaluation of both Safety Programs. Numbers could be different if separated by Utility and City. All evaluations are available for you viewing. 1. How is the Safety Program with raising the Safety awareness for you? 8.4 2. What do you like/dislike about the Safety Program? 8.0 3. Rate the Quality of the Training presentations and the materials? 8.7 ((ex.)Videos, Power Point,Games, Group discussions, Presentation or Presenter) Please Comment on what works and other ways to do them. 4. How committed is your employer to Safety? 9.2 5. How Committed is your immediate Supervisor to Safety? 9.0 6. How do you rate your MMUA Safety Coordinator? 95 7. Please suggest 3-Safety Topics that you would like covered or covered in greater depth. 1) CDL Information, OJT/hands on per Department 2) Stress Management, Harassment&MSDS in Office 3) More Supervisor Training Overall Rating: Very Good Comments: First,the support and participation from the Utility has been very good. The Utility Departments have been tremendous to work with. This past year we have worked on the Respirator and Hearing Programs. Accident procedures were evaluated and upgraded. During work site observations the crews have exhibited an understanding of what is needed with respect to briefing the entire crew, controlling the work zone and using proper procedures for energized & de- energized work You will be establishing a Hearing baseline and I have completed various noise surveys for a variety of equipment. We have developed Driver Training program (St. Cloud site)that will include all of the staff using the Utility vehicles. Please continue to require Contractors to follow the OSHA standards as a minimum. Contractor Safety is always a concern. Remember to include Safety as a topic in all Pre-Construction meeting and in Staff meetings. The Electric Department is very Pro active. The mentoring that you have developed for the Electric Department is a model for all Employers. The growth of your Water Department was needed due to the amount of work required. The growth of the community is creating more and more pressure on your Departments. It is essential that during this growth, Safety be not overlooked. Glenn Sundeen is tremendous and is a role model for Electric Supervisors. I appreciate the opportunity to work with you, your Staff and your Safety Program.
