1. Approve,by motion, a massage establishment license to Life's Retreat Massage and Spa, Inc., 570
Dodge Avenue NW,valid until December 31, 2014.
2. Approve,by motion,massage therapist licenses to the following individuals to perform massage at
Life's Retreat Massage and Spa,valid until December 31, 2014:
• Kayla Brown
• Tara Gardner
• Alythia Sully
• Dawn VantHof
Background/Discussion
Life's Retreat Massage and Spa, Inc.,has applied for a massage establishment license for 570 Dodge
Avenue NW.The individuals listed above have applied for massage therapist licenses to perform massage
at Life's Retreat Massage and Spa.
All applicants have successfully cleared Police Department background investigations and the
establishment has successfully completed inspections by the Fire and Building Safety Division.
