Massage Therapist License
Applicant
Daniel Robert Guthrie
Backuround
The applicant is requesting a massage therapist license to practice therapeutic massage at
Avalon Salon located at 706 Main Street. The applicant has met the 500 hours of completed
course work and the license will be contingent upon successful clearing of a background
check. The applicant will be working as an independent contractor at the salon and therefore
must also provide proof of insurance in an amount not less than $500,000.
Action Requested
Council motion approving a therapeutic massage license to Daniel Guthrie contingent upon
the following:
· city receiving certificate of insurance in an amount not less than $500,000
· applicant passes a background check to be completed by the Police Department
