Discussion
Staff would like to change the look of the Request for Council Action form. Attached for
your review is the new form. City Administrator Lori Johnson and I reviewed several other
cities' forms and designed this form to meet our needs. We felt that the flow is better
making it easier for both Council and staff in reviewing the information.
