Dear Bruce,
This evaluation is for 2005 and is a combination of employee /management actions and
reactions throughout the past year and my observations of them. Also, it is a review of
OSHA and DOT Safety compliance of your program. The SP-9 evaluation format that I
am using is detailed in your A W AIR Program.
1. Employer Commitment:
. Good Pro-active. Safety is a concern.

2. Employee Involvement:
. Good attitude. Very Good participation.
. The City Safety Committee has been very Pro-active.
3. Hazard Control:
. Documented walk through self-inspections are done on a regular basis.
. Written SOP and guidelines are suggested for major tasks.
4. Facility Maintenance:
. Gary Lore and his staff are tremendous assets. Having so many buildings
there are many concerns and they are being addressed as needed.
5. Accident Analysis:
. Accidents are being reviewed.
. Near misses are identified and reviewed at Safety meetings.
6. Occupational Medicine:
. Is complete as needed.
7. Contractor Safety:
. We need to continue to inform contractors of their Safety responsibilities.
8. Employee Training:
. Compliance Training is on pace and documented.

9. Emergency Response:
. All Departments kept well informed.
Overall Rating: Very Good
Comments:
This past year we have worked on:
1. Blood Borne Pathogen Manual
2. Blood Borne Pathogen Exposure Packet
3. Lockout I Tag out Manual
4. Natural Gas Visiting speaker-Terry Crooker Center Point Energy
5. MN OSHA mock inspection has been completed at an sites
6. Accident Investigation was emphasized
7. CDL Presentation from MN DOT (Dick Norberg)
8. Sexual Harassment Awareness & Prevention Training-League ofMN Cities
9. An update was distributed on Crisis procedures and dealing with the Media.
First, the support and participation from the different Departments has been very good.
During work site and shop observations the various crews have exhibited an
understanding of what is needed with respect to briefing the entire crew, controlling the
work zone and using proper procedures.
Please continue to require Contractors to follow the OSHA standards as a minimum.
Contractor Safety is always a concern. Remember to include Safety as a topic in all Pre-
Construction meeting and in Staff meetings.
The growth of the community is creating more and more pressure on your Departments.
It is essential that during this growth, Safety be not overlooked. I appreciate the
opportunity to work with you, your Staff and your Safety Program.

This comment is a question I am asking all locations I travel to:
This is a question I want you to pose to yourself.
It is a concern that I have noticed in my years of visiting many different locations.
Have you as the manager recently walked through your various Departments without a
specific reason? "Just to see how things are going!!!1 1"
To often the busy schedule that you have becomes so overwhelming so that you do not
have time to check on your "MVR" (most valuable resource-your staff)
This doesn't apply to everyone but if it does----See if you can make an attempt.
. Lastly, communications is the weakest link in all organizations.
If at all possible make sure to have manager staff meetings on a regular basis. If you are
currently meeting----continue; if not try to get some form of communication so that all of
your managers work on pulling in the same direction.
We have made progress and need to continue.
