The applicant is requesting a massage therapist license to work at Salina Salon & Day Spa located at
19262 Freeport Street.
Discussion
The applicant submitted a transcript showing she has completed the 500 hours of coursework that is
required by the city. She also cleared a background check by the Police Department.
The applicant has not yet received her diploma. She stated it would take approximately six weeks to
receive it from the school. Per our ordinance, the city requires a copy of the diploma.

Action Requested
It is recommended that the City Council approve a massage therapist license for Amanda Larson to work
at Salina Salon & Day Spa beginning February 6, 2007, on the condition that she must submit a copy of
her diploma by April 1, 2007. If the city does not receive a copy of the diploma, her license will become
void as of April 2, 2007.
