Introduction
The applicant would like to transfer her massage establishment and massage therapist license to 200 5'h
Street, Suite D.
Discussion
The applicant has submitted all the necessary application materials. The building and planning department
have signed off on the new location.
The fire chief has yet to review and sign off on the application.
The applicant would like to begin work at the new location this Friday, April 20, 2007 but there will not
be another regular Council meeting until May 7.
Action Requested
Council motion for a location transfer of a massage establishment and massage therapist license to
Delores Matz-North Country Massage to 200 5'h Stteet, Suite D, contingent upon the applicant meeting
all fire codes.
