Massage Therapist License Application
APPLICANT
· Debbie Donald-Avalon Salon
BACKGROUND
The applicant is requesting a massage therapist license to practice therapeutic massage at
Avalon Salon located at 706 Mare Street in the City of Elk River. The applicant has met the
required 500 hours of completed course work. At the time this memo is being written, the
Police Department is running the required background check, therefore, approval should be
contingent on a successful background check.
ACTION REQUESTED
Council motion approving therapeutic massage license for Debbie Donald contingent on a
successful background check.
