Massage Therapist License Application
APPLICANT

BACKGROUND
The applicant is requesting a massage therapist license to practice therapeutic massage at
Moe Therapeutic Massage located at 934 Highway 10, in the City of Elk River. The applicant
has met the required 500 hours of completed course work. The Police Department is in the
process of conducting the requited criminal history background check at this time.
ACTION REQUESTED
Council motion approving therapeutic massage license for Daisy English to practice
therapeutic massage at Moe's Therapeutic Massage contingent upon a successful background
check.
