January 19, 2003 Chief Tom Zerwas Elk River Police Department 13065 Orono Parkway Elk River, MN 55330 Dear Tom, This letter will serve as a notice of resignation for my position. I just accepted a position with the HealthEast Care System as a Public Access Defibrillator (PAD) Coordinator. I am excited about the new challenges that this position has to offer and look forward to participating in a program that I feel strongly about. Unfortunately, I am unable to give more than two weeks notice as HealthEast requested an immediate start date. My last day will be February 2, 2003. So as not to leave in the middle of some of my tasks, I recently completed a medical supply order, revised the department's blood born pathogen exposure report form, and already applied for this year's training and reimbursement grant through the Central MN EMS Region. I will organize and explain these duties to whomever you wish and will be available at any time in the future to assist with any questions that person may have. I am proud to have been a part of the Elk River Police Dept. for the past 9 ~ years and have enjoyed watching the community and the department grow. I will truly miss all of the wonderful people I have met and the dear friends that I have made. I would also like to thank you personally for making my time with the department an enjoyable and memorable experience. Sincerely, Linda Ellingworth
