Massage Therapist License Application
APPLICANT
· Melanie Lundberg
BACKGROUND
The applicant is requesting a massage therapist license to practice therapeutic massage at an
establishment which she owns. The establishment is "Massage by Melanie" and is located at
592 Dodge Avenue in the City of Elk River. The applicant is submitting a renewal license as
she was currently licensed in Elk River and practicing at another location. She has met the
required 500 hours of completed course work.
ACTION REQUESTED
Council motion approving therapeutic massage license for Melanie Lundberg to practice
therapeutic massage at "Massage by Melanie."

REQUEST
License for a therapeutic massage establishment
APPLICANT
Melanie Lundberg - 592 Dodge Avenue, Elk River, MN 55330
BACKGROUND
Melanie Lundberg is requesting a license for a therapeutic massage establishment at 592
Dodge Avenue in Elk River. The applicant has submitted all of the required application
material. The Police Department is in the process of processing the required background
check. Therefore, approval should be contingent upon a successful background check.
ACTION REQUESTED
Council motion approving a therapeutic massage establishment license for Massage by
Melanie, 592 Dodge Avenue, Elk River, MN contingent upon a successful background
check.
