Dear Bryan,
This evaluation is for 2005 and is a combination of employee /management actions and
reactions throughout the past year and my observations of them. Also, it is a review of
OSHA and DOT Safety compliance of your program. The SP-9 evaluation format that I
am using is detailed in your AWAIR Program
Employer Commitment:
• Very Pro-active. Safety is a concern.
2. Employee Involvement:
• Good attitude. Good participation.
• The City Safety Committee tries to be very Pro-active and the Utility
representation is a great asset.
3. Hazard Control:
• Documented walk through self-inspections are done on a regular basis.
4. Facility Maintenance:
• Concerns are addressed as needed.
5. Accident Analysis:
• Accidents are being reviewed.
• Near misses are identified and reviewed.
6. Occupational Medicine:
• Complete as needed.
7. Contractor Safety:
• Continue to inform contractors of their Safety responsibilities.
8. Employee Training:
• Compliance Training is on pace and documented.
9. Emergency Response:
• All Departments kept well informed.

Comments:
This past year we have worked on:
1. >~lood Borne Pathogen Manual
2. Blood Borne Pathogen Exposure Packet
3. Lockout /Tag out Manual
4. Natural Gas Visiting speaker-Terry Crooker Center Point Energy
5. MN OSHA mock inspection has been completed at the Plant and Shop
6. Accident Investigation was emphasized
7. CDL Presentation from MN DOT (Dick Norberg)
8. Sexual Harassment Awareness & Prevention Training-League of MN Cities
9. An update was distributed on Crisis procedures and dealing with the Media.
First, the support and participation from the Utility has been very good. The Utility
Departments have been tremendous to work with. This past year has been good for your
Utility. As a management staff you were able to visit Shakopee Utilities and hopefully
exchanges of ideas will follow.
Please continue to require Contractors to follow the OSHA standards as a minimum.
Contractor Safety is always a concern. Remember to include Safety as a topic in all Pre-
Construction meeting and in Staff meetings.
The growth of the community is creating more and more pressure on your Departments.
The Electric Department has been and continues to be very Pro active. You have had a
very good managerial staff with Glenn and Dave. You have added a tremendous asset in
Theresa as Office Manager and the reassignment of Dolores to a HR role will be great.
On the addition of Theresa, she has picked up various duties that are required for her
position such as the Substance Abuse program, CDL rules and OSHA 300 reporting as
fast or faster than anyone I have worked with.

Required to be prepared by and signed off by a PE.
This comment is a question I am asking all locations I travel to:
This is a question I want you to pose to yourself.
It is a concern that I have noticed in my years of visiting many different locations.
Have you as the manager recently walked through your various Departments without a
specific reason? "Just to see how things are going! ! ! ! !"
To often the busy schedule that you have becomes so overwhelming so that you do not
have time to check on your "MVR" (most valuable resource-your staff)
This doesn't apply to everyone but if it does----See if you can make an attempt.
Lastly, communications is the weakest link in all organizations.
If at all possible make sure to have manager staff meetings on a regular basis. If you are
currently meeting----continue; if not try to get some form of communication so that all of
your managers work on pulling in the same direction.
It is essential that during this growth, Safety be not overlooked. I appreciate the
opportunity to work with you, your Staff and your Safety Program.
