ERMUSR 03-08-2005~~~„~'~ riiriv~~ Minnesota Municipal Utilities Association Elk River Public Utility 13069 Orono Parkway Elk River, MN 55330 Mr. Bryan Adams February 9, 2005 Dear Bryan, This evaluation is for 2004 and is a combination of employee /management actions and reactions throughout the past year and my observations of them. Also, it is a review of OSHA and DOT Safety compliance of your program. The SP-9 evaluation format that I am using is detailed in your AWAIR Program. I have added a Program evaluation to the process given in the month of January 2005. First evaluation is the (SP-9) review of your Safety Program consisting of 9 elements. The following is an overview of the Safety categories and your Safety Program evaluation in each category by me. Employer Commitment: • Very Pro-active. Safety is a concern. 2. Employee Involvement: • Good attitude. Good participation. • The City Safety Committee tries to be very Pro-active and the Utility representation is a great asset. 3. Hazard Control: • Documented walk through self-inspections are done on a regular basis. 4. Facility Maintenance: Concerns are addressed as needed. The addition of Chris as a Warehouse worker has helped with organization and area clean-up. Accident Analysis: Accidents are being reviewed. Near misses are identified and reviewed at Safety meetings. 6. Occupational Medicine: • Complete as needed. 7. Employee Training: • Compliance Training is on pace and documented. • Additional training for your staff has been a priority and is the way to maximize you greatest resource. 9. Emergency Response: • All Departments kept well informed. The second means of evaluation is the MMUA Safety Program Evaluation /Comment Questionnaire (completed Jan. 2005). Your Staff rated the following questions and here are the averages for each topic. All evaluations are available for you viewing. Due to the way the evaluation was distributed and collected, it is an evaluation of both Safety Programs. Numbers could be different if separated by Utility and City. All evaluations are available for you viewing. Some of the suggestions from the following survey are: --Mix up the hour with games, small groups, role play situations and interactive activities. --Use of props, displays, hands on and videos to support the training. --More specialized training for the Various Departments. --More examples of concerns that are at Home as well as work. --Visiting speakers are good. Training Methods (PowerPoint, Discussion, Lecture, Games), Time of day, Length of training, Training Topics Please use following area for your comments: Overall Rating: Very Good Comments: First, the support and participation from the Utility has been very good. The Utility Departments have been. tremendous to work with. This past year we have worked on the AWAIR program manual, the Respiratory program, the Excavation manual and the individual Hearing results. The Hearing Conservation Program included charting each staff's Hearing test and explaining what the results mean. During work site observations the various crews have exhibited an understanding of what is needed with respect to briefing the entire crew, controlling the work zone and using proper procedures for energized & de-energized work. The Program evaluation completed in Jan. of 2004 suggested guest speakers. "This past year we had MN OSHA Consultation talk about Workplace Violence and Olson Co. demonstrate chain and sling inspections. We have developed a Driver Training program (St. Cloud site) that has been well received by all who have attended. Due to concerns for Utility security, you sponsored a presentation on that subject. It was an eye opener for many of your staff members. Please continue to require Contractors to follow the OSHA standards as a minimum. Contractor Safety is always a concern. Remember to include Safety as a topic in all Pre-Construction meeting and in Staff meetings. The growth of the community is creating more and more pressure on your Departments. The Electric Department has been and continues to be very Pro active. The growth of your Water Department has created more challenges for you and the Water Department Supervisor. Dave Berg is a very good man and I know that he has the ability to handle the challenges. It is essential that during this growth, Safety be not overlooked. I appreciate the opportunity to work with you, your Staff and your Safety Program. We have made progress and need to continue.
