1. Approval of a massage establishment license for Collyard Chiropractic, PA located at 510 Freeport
Ave, Suite F.
2. Approval of massage therapist licenses for Laura Devine-Vollkommer and Tara Lefebvre to work at
Collyard Chiropractic, PA.. Ms. Lefebvre’s approval should be contingent upon receipt of proof of
graduation.
Background/Discussion
All necessary application materials have been received and all applicants have successfully completed
Police Department background checks.
